Does not allow for company name of landlord. Does not allow for a change in ownership of property since original lease was signed. otherwise, very easy to use.

This form needs additional options. There is no option for a month to month verbal agreement. There was not a signed lease and the tenant is a family member. However, Illinois law requires that anyone maintaining residency for a certain period of time must be evicted in order to make them leave the property.

The only issue was that there was no lease signed. This was an oral agreement by the former owner (my grandmother) and the tenant. I had to make up dates to process the form and now will actually have to manually edit the letter before sending.
